Lichen planus is a self-limiting condition that usually produces an itchy rash that is fairly similar on both sides of the body; a symmetrical rash. Skin rash is most common, but 15-30% have changes in the mucous membrane. Nails can also be affected. Some also get a rash in the abdomen. Erosivlichen planus is a rarer subtype with painful ulcers. Figures for occurrence are uncertain, but it is estimated that lichen planus occurs in 1% of the population and is most frequently seen in 30 to 60 year olds. Lichen planus also rarely occurs in children.
The cause is unknown. A lichen planus-like rash can be triggered by drugs and the doctor will therefore survey the drug history.
If treatment is felt to be necessary, cortisone cream is the first choice, but in the case of a widespread rash, tablet treatment is appropriate, most often cortisone or acitretin is chosen. Light treatment by a dermatologist is also used. Antihistamine tablets can also help with the itching. Most people get rid of the rash within a couple of years.
